Puppypalooza Event Waiver Creator

We aim to give fans a world-class experience at Progressive Field bringing them closer to the action than anywhere else. Safety is a top priority for the Cleveland Indians and this special event requires a waiver before participation.

This Event Waiver Creator tool creates a waiver for you to print and bring with you to the event. PLEASE NOTE: Entering data in this form DOES NOT submit your waiver to the Cleveland Indians.
